Ms Hannah Reed Community Nursing Service 21 Willow Road Westford 30 October 2026 Dear Ms Reed, Re: Mr Daniel Cole, DOB 12 February 1958 I am writing to request community nursing support for Mr Cole following his discharge today after treatment for a chest infection. Please review his recovery and help him follow the care plan at home. Mr Cole was admitted with a productive cough, fever and reduced exercise tolerance. His symptoms have improved with treatment, and he is now able to walk around the ward without assistance. He remains tired after longer distances and would benefit from a gradual return to his usual activities. Before discharge, we discussed his prescribed medicines and checked his understanding of the instructions. He demonstrated the correct inhaler technique and was given a written plan. His daughter will collect the medicines and visit him each evening, although she is unable to assist during the day. Please arrange an initial home visit to assess his breathing, medicine use and ability to manage daily activities. He has been advised to seek urgent medical attention if his breathing worsens or he becomes confused. A review with his general practitioner has been arranged for next week. Please contact the ward if you require further details about his admission or discharge plan. Yours sincerely, Registered Nurse
